The behind-the-kneecaps method Brandon mentions works very well, at least with lighter spinning and fly rigs, and I have been using it for many years. It provides so much leverage that I found, as a young man, my hands would slip. In later years, I find that my grip is the limiting factor.
I would think that in extreme cases, combining the neoprene and going behind the knees would be the way to go.
