What a strange lure! It is meant only for a two treble set up. You choose either the tail or the swivel just below. Not both

This seems pretty intelligent to me.
I guess the middle eyelet is meant for rigging the lure with only one hook as this doesn't affect the balance of the lure as much as having only one hook at the tail eyelet.
